Dr. Ryan Crenshaw is NOT a participating provider with the following insurance plans:
- Medicare and Medicare replacement plans

- Amerigroup (unable to accept as a self-pay patient)
- Anthem HealthKeepers Plus
- Anthem HealthKeepers Pathway Tiered Hospital or Pathway X Tiered Hospital ID Numbers that begin with: YTO/YTR/YTU/YTY/XTO
- United HealthCare Marketplace Silver Compass
If Dr. Crenshaw is not a participating provider with your insurance plan, you will still be welcome to make an appointment as a self-pay patient. Under these circumstances, you will be asked to pay for your office visit prior to the encounter. Same would apply if a procedure is recommended.
